Medical Examination review
In some cases, the child will also be required to have their medical condition reviewed to determine whether they still meet the medical qualifications for Child Disability Allowance.
The medical practitioner or nurse practitioner completing the initial application will indicate whether or not the child's condition needs to be reviewed at a later date.
The Medical Examination review form will be issued by SWIFTT 8 weeks before the medical review date. This allows time for the client to visit their medical practitioner or nurse practitioner for a medical examination.
Medical Examination form indication
If the Medical Examination form indicates:
- that the child's medical condition has been reassessed with a future date, then extend the medical expiry until the date indicated by the health practitioner
- that the child's medical condition has not been reassessed with a future date, then cancel the Child Disability Allowance with the expiry date on the current Medical Examination form
