Authorised agent

If a client chooses to appoint an authorised agent to act on their behalf they can do so by completing an agent form or by informing the Ministry of Social Development in writing.

When the agent form or the written request is received, you should do the following:

• check the signatures of the client and agent
• check what information the agent is allowed to access
• check how long the agent has been appointed for
• keep a copy of the written consent or Agent form in the client's file
• make a note on CMS under "Service Details - agents details" with the agent's name and their role as agent

Note before disclosing the client's personal information to an agent you must check that, the agent has authority to obtain the client's personal information.
