Application for assistance required

(1)

MSD must not grant assistance under this programme until MSD has received-

(a)

an application for assistance completed by or on behalf of the applicant to MSD's satisfaction, in such form as the chief executive from time to time prescribes; and

(b)

any supporting evidence reasonably required by MSD.

(2)

MSD may waive all or part of a requirement to provide information (including information specified in an application form) under subclause (1) if satisfied that MSD-

(a)

already holds the information concerned; or

(b)

already holds enough other information to determine the matter for which the information concerned is needed.
