Canadian Life Certificates

When a client is receiving a Canadian pension in New Zealand the Canadian authorities require them to complete life certificates.

The Canadian authorities will send the life certificate form directly to the client when it is due.

The client should complete the life certificate and return it directly to Canada.

If a client needs the life certificate witnessed it is important to check that:

• the life certificate is fully completed, signed and dated
• the client's address has been verified and
• the certificate has been stamped with the office date stamp in the presence of the client

Return the life certificate to the client so that they can send it directly to the Canadian authorities.
