Applications in review forms

The Ministry of Social Development issue review forms to gather information to review a client's current entitlement.

The information that is submitted by the client in the review form may contain information that is essentially a request for assistance other than a benefit for a particular need or purpose (ie an application). Where this is the case, this must be regarded as an application and investigated by the Ministry.
