Verification

You must be satisfied that you can accurately identify the deceased client's (or child's) record.

Generally, we will accept verbal notification of a client's death. This advice can be acted on when you determine the relationship of the caller to the deceased client and obtaining sufficient information to provide confirmation of the deceased's identity.

Follow-up verification should generally be requested. A copy of the death certificate or a copy of the death notice is acceptable support documentation.
