How to apply
A client should complete the Application for Civilian Amputee Assistance form and provide an appointment card or certificate of attendance from the limb centre.
For dependent children the application form should be completed by the parent, guardian or primary caregiver and payments should be made on parents, guardian's or primary caregiver's record.
Evidence of costs
Clients will need to provide:
- estimates of their costs if applying before they attend the Limb Centre or
- verification of their actual costs once the appointment has been completed
