Client transfers to a new service provider

If a client transfers from one service provider to another, their Community Costs can be paid provided that:

• they complete a new Community Costs Payment application form
• the client meets the qualifications for Community Costs
• they have costs in the community and
• the treatment programme is not intended to be more than 39 weeks
