Personal advisors role
As part of the Transition from Care to Independence pilot, each young person will have a personal advisor contracted by Oranga Tamariki. The personal advisor will help develop a pathway plan and provide support to access a range of programmes and services available to them.
The personal advisor will begin their involvement with the young person:
- before discharge
- during the transition stage and
- support post discharge
The personal advisor will prepare the young person for their discharge from supported care and refer the young person as a participant on the pilot.
Personal advisors will work in partnership with the young person, their social worker and caregivers to develop and review transition plans to ensure the young person is given sufficient opportunities and support to develop independent living skills.
The role of the personal advisor is to:
- mentor
- be a contact point for the young person until they reach 19 years old
- help in accessing safe and affordable accommodation
- help the young person develop life skills
- send them on personal development services
