Helping your client obtain existing assessments

Generally we expect that your client will have access to existing assessments and will bring these along when they apply for Supported Living Payment.

If your client is unable to, or it is not reasonable to expect your client or their parent, guardian or agent to find this information you can assist them to do this.

The client consent to release of health or disability information process allows us to give assurance to third party holders of medical or disability information that they are able to provide us with this information under the Privacy Act.

If your client asks you to assist them to obtain their medical or disability information from a third party, they must sign the consent form. Third party providers may be more satisfied that they are able to provide the information to us if the provider specific section is completed.
