13 | Applications | |
13.1 | An application for a Payment must be made on a form provided by MSD. | |
13.2 | An application for a Payment must be made by the Applicant, except where- | |
(a) | the application is made in respect of a Child; or | |
(b) | the application is made by a person on behalf of another person if the person who would otherwise make the application is unable to do so because of sickness or disability. | |
