New OSCAR centres

The following proof is required from any new centre that wants to be paid an OSCAR subsidy:

• proof that they are an approved centre (available from Ministry of Social Development Approvals)
• proof of bank account
• details of their postal and residential address
• service providers name, phone number and a copy of their signature
• fee structure which includes any centre closures over the year

Fax these details to Data Integrity, who will then enter the information on SWIFTT.

Ensure a file is set up for the centre, a liaison officer is assigned and forms and pamphlets are made available.

Under no circumstances can we pay a subsidy to a provider that is not approved.
