Residential Support Subsidy : Contents
Application process
All residential service providers in New Zealand are required to use the Residential Support Subsidy authorisation form to advise Work and Income when a client in their care has been assessed and will be subsidised by the Ministry of Health.
The Residential Support Subsidy Authorisation Form is available online.
When the residential service provider fully completes the form, providing full details of the assessment for residential care, it is accepted that:
- authorisation has been given by the Ministry of Health
- the client is in the target group and meets the qualifications for Residential Support Subsidy
- their service co-ordinator has arranged a suitable care facility
- the service provider is contracted by the Ministry of Health
- Ministry of Health funds are available to pay for the cost of the client's care
and
- if the client is getting a benefit they must contribute towards the cost of their care
